There are three ways to get your biometric screening, and you may choose the easiest option for you.
Screening at a LabCorp location
- Go to www.eHealthScreenings.com/login to obtain a LabCorp Screening Form (use screening key MAP07)
- Register for an offsite screening at a LabCorp location
Screening with your own doctor
- Go to www.eHealthScreenings.com/login to obtain a Physician Screening Form (use screening key MAP07)
- Complete a biometric screening with your physician
Participate in an onsite screening
- Go to www.eHealthScreenings.com/login to register for an appointment (use screening key MAP07)
Health Check Survey
Complete a confidential Health Check Survey in the Better Health Wellness Portal powered by Virgin Pulse.
- Register at join.virginpulse.com/BetterHealthProgram
- Under the Health menu, select Surveys, and find the Health Check Survey
- If you already completed this survey during 2023, you will need to do it again between October 1 and December 6 to qualify towards the premium discount.
- This survey is completely confidential; MAPFRE does not receive any of your data, other than confirmation that you have participated

Get started at www.eHealthScreenings.com/login. If you participated last year, you can use the same username and password. If you forgot your username or password, use the links provided to recover your information.
If this is your first time, click “Register Now” and follow these steps:
- Use screening key MAP07
- You will be asked for a default username and password; use the following format (note: these are case sensitive):
- Username: First initial + last initial + date of birth in MMDDYYYY format (for example, John Smith born 4/23/75 is “JS04231975”)
- Password: Last name + last four digits of your SSN (for example, John Smith with SSN 123456789 is “Smith6789”)

Submitting Your Screening
You must complete your screening and Health Check Survey by December 6, 2023.
If you got your biometric screening from your doctor, you can:
- Upload your biometric screening form to the eHealthScreening portal
- Email your biometric screening form to ehs.physicianscreening@ehealthscreenings.com
If you have any issues uploading or sending your form, call eHealth at 888-708-8807.
If you are an employee covered under the 2024 medical plan, and complete and submit your biometric screening and Health Check Survey by December 6, 2023, MAPFRE will automatically adjust your premium to the discounted rate. You do not need to take any further action.

What is a biometric screening?
A biometric screening is a clinical test conducted by a trained lab provider or physician. The provider will collect several measurements, including height, weight, blood pressure and a blood sample. The screening typically takes less than 15 minutes. The blood sample will test for your total cholesterol, HDL and glucose levels and is a great way to learn about potential health risks.
Do I need to fast before screening?
For lab screenings, fasting is recommended, but not required. Water is permitted and encouraged. Diabetics should consult with your physician before fasting. Participants should continue to take all prescription medications.
What can I expect when I go to a lab?
It’s preferable that you set up an appointment time with the lab. Depending on the lab location, appointments may be required. The technician will take your biometric measurements and a blood draw. You will receive notification within 3-5 days that your results are posted on the EHealth portal.
How quickly will my physician screening form be approved?
If you choose to receive a screening from your own doctor, the forms are typically processed within 24-48 business hours. You will be notified if there is information missing or if your submission is complete.
Does the program cost me anything?
You do not need to pay for the screenings and annual physicals are covered at 100% as part of your medical plan preventive benefits.
Do I have to participate in the program?
No. The program is completely optional. We encourage you to participate to learn more about your health status.
How do I access my LabCorp results?
You will be granted access to an online health profile on the eHealthScreenings portal and your results will be posted within 3-5 days of your screening.
What should I do with my results?
Results from the LabCorp screenings will not automatically be provided to your physician. Review your results on your own and decide how you want to act. We encourage you to reach out to your physician and share your results at your next visit.
What is the discount? When will I receive it?
You will only receive the discount if you are covered under the MAPFRE medical plan in 2024 and you completed all steps by the December 6 deadline. The discount will begin in the first pay period of 2024. The discount amount is $520, which will be pro-rated over the 26 pay periods in 2024 – or $20 per pay period.
Do I receive the rest of the discount if I leave MAPFRE?
No, you will only receive the discount up to the date you were actively employed. The difference will be forfeited.
What type of privacy measures are being taken?
All data is securely transmitted and kept confidential in your personal health profile. eHealthScreenings is required by law to maintain the privacy and security of your personally identifiable health information.
Are my results confidential?
Yes. All information collected as part of the biometric screening process is Protected Health Information (PHI) and secured according to the Health Insurance Portability and Accountability Act (HIPAA). MAPFRE will not have access to any individual results.
Is MAPFRE receiving my biometric data and results?
No. Please see above. MAPFRE will not have access to any individual results. Only aggregate (full population participation) data will be shared back with MAPFRE to help inform future wellness program options for employees. Additionally, names of those who complete the program will be reported back to allow MAPFRE to apply the discount in 2024.
How do I complete the Health Check Survey?
